Medical Information for Trojan Student-Athletes
Welcome to Hannibal-LaGrange University! We are excited to have you and look forward to watching you compete this upcoming season!
The Athletic Training Department utilizes a secure, online data entry system (SportsWare) for all of our medical forms. All student-athletes must complete their entire SportsWare profile and have a physical exam prior to participating in any team practice (including weights, conditioning, etc.) or competition! Hard copies of the forms are only accepted if the student-athlete is a minor! In this case, please see the “Minor Packet” below.
The athletic training staff partners with physicians in Hannibal to offer a free physical to our student-athletes. There are two physical dates in August of each year – one for fall sports and one for winter/spring sports. These dates will be announced during the summer. Should you utilize your own physician at home, you must use the physical form located in SportsWare – this is the ONLY physical form accepted! All new athletes (freshmen and transfers) MUST have a full physical before being allowed to participate!

Information for Visiting Teams
The Hannibal-LaGrange University Athletic Training Staff looks forward to hosting your team at our upcoming athletic event!
Our Athletic Training Department is staffed with a full-time, certified athletic trainers. Natalie Geralds, our Head Athletic Trainer, works to oversee the medical care for all varsity and junior varsity sports at HLGU.
Our main athletic training facility, the Trojan Athletic Training Center, is located just southeast of the Mabee Sports Complex (MSC) in the grey building. This facility will be open for men's and women's soccer, men's and women's cross country/track and field, men's and women's golf, baseball, and softball competitions. The MSC Athletic Training Room is located on the first floor of the MSC just off of the wood court. It will be open for men's and women's volleyball, men's and women's basketball, and wrestling.
The respective athletic training facility will open at least one hour before a scheduled competition. Available modalities include: moist heat packs, cryotherapy, whirlpools, and (with proper documentation from your school's certified athletic trainer or team physician) electrical muscle stimulation and ultrasound. We do ask that each team bring a properly stocked medical kit for any taping needs. The athletic training facilities are equipped with emergency equipment, including: AED, spine board, vacuum splints, slings, crutches, and first aid items. Please contact Natalie Geralds for any special needs that your athletes may need at least 24 hours before your team arrives.
In the event of an emergency, athletes will be transported to the emergency room at Hannibal Regional Hospital (HRH), 6000 Hospital Drive, Hannibal, MO 63401. The hospital phone number is (573) 248-1300, for Emergency Services: (573) 248-5100.
Directions from MSC to HRH:
- Turn LEFT onto MO-168 E
- Take 1st LEFT onto McMasters Ave
- Turn RIGHT and merge onto US-63 W toward Monroe City
- Take the Shinn Ln exit
- Turn LEFT onto Co Rd 437/Shinn Ln
- Turn RIGHT and follow signs to Emergency Room
